Medicine consent form for parents. Please print a hard copy (hard copies available from letter rack in the office) and sign to hand in with medicine at the office. ALL MEDICINES MUST BE LABELLED WITH YOUR CHILD'S NAME AND MUST BE IN THE ORIGINAL CONTAINER.

We are happy to hold and administer an inhaler in school for your child if they have asthma. If you need us to do this for you then please complete and return the Chlidren's Asthma Plan. You will also need to complete the Administer Medicines Form so that we can have your clear instructions on how you would like us to administer this medication for your child. Please note that we will need an inhaler that is in date and can be kept in school at all times. Thank you. Can you leave all medications with the office only - thank you.
